Let us know the details of the DME equipment or supplies you need and the name of your doctor, and we will take it from there. We will reach out to your doctor to confirm the DME order and request other details required by your insurance plan.

Things you need to know about your new pharmacy

Please ask your doctor to send Prescriptions to 'Qwark'. They can look up Qwark in their systems with the zipcode: 23059 or the NABP number: 4850637. Alternatively, they can fax the prescriptions to 804-508-6634. Finally, they can call the pharmacy line directly at 804-601-4122.

No. Your copay and out-of-pocket maximum stay the same as before. Qwark will help you with copay assistance, when available, and look for the best price always, but switching to Qwark does not impact your plan-defined copays in any way.

Most likely, yes. Qwark offers over 15,000 brand and generic medications. If your prescription is covered at your current pharmacy, it will be covered at Qwark. However, whether a new prescription/drug is covered or not is decided by your plan, not the pharmacy. If it's covered by your plan, Qwark will have the medication in stock for you!

Absolutely! We carry all acute medications and fill most of them within a few hours. We strongly recommend that you keep all your prescriptions with Qwark, so you can manage prescriptions in one place.
